Truthfully—though he’d never admit it anymore because of the way people reacted when he said it—he’d also always felt like he’d gotten away from Sloan a little too easily. He hadn’t had to kill Sloan, or blackmail him, or any of the other dark fantasies that had seemed like his only options for escape at the time. He hadn’t even had to find the courage to confess his shameful situation to a friend and ask pitifully for their help. And after all the threats Sloan had made about revealing Julian’s most damning secrets if he tried to leave him, it was almost disconcerting that Sloan hadn’t followed through on ruining his life. He wasn’t the type to break his promises of violence.
But now it all made sense. Sloan had just been biding his time.

Summary
Julian shoots Garak in the holosuites to prevent him from endangering the command crew of Deep Space Nine. In some ways this changes nothing, because Julian had never tried to hide his dedication to Starfleet’s principles. In other ways this changes everything, because Garak has never cared to interrogate his perception of the sweet, awkward, innocent doctor who has always been so gratifyingly gentle with him.

After Extreme Measures, Julian struggles with experiencing Sloan's memories as his own. The more he dreams, the more the memories take their toll, especially when he starts seeing encounters with himself through Sloan's eyes that he doesn't remember.
