The form is a single page, double-sided, and it asks the parties to describe the nature of the conflict in neutral language and to identify three potential resolutions.
Dr. Geto and Dr. Gojo have never filled out this form.
The administrator who handles this form stopped expecting them to at around the four-month mark, approximately six years ago.
