Dean wished he knew what was wrong, but Sam didn’t say a word, he just moved as if on autopilot, his gaze blank and stuck on the floor and the tears falling silently."
Dean and Mary rescue Sam from Toni and the British men of letters, but with Cas mysteriously MIA, Dean falls back into his old routine of patching Sam up himself, leaving Mary feeling like an outsider to their little family. Only Dean's much better at physical wounds than emotional ones, and the stitches aren't what Sam needs his help with.

Steve could tell he was not in fact fine at all. He was pale and constantly shaking, almost like he was cold. Beads of sweat dotted his forehead, and his gaze was vaguely unfocused. He looked haunted. And weak. Even with Steve carrying most of his weight it was slow going. He was tempted to pick Bucky up again— one, so they could get away from that hellhole that had done this to him, and two, so he could hold him close.

Sam’s blood is sticky on his fingers as he feels the wound and tries to convince Sam, already fading, that everything will be okay, because it has to be.

“Whoops,” Sam says casually. A thread of tension stretches between them, almost interminable, and then they both move at once. The demon in Sam’s body slams Dean back, and Dean crashes against the bar.
