So now he, and by extension, Ossan, have to deal with a Hollowfied Ichigo stuck in some sort of dissociative state— oh crap now they're all stuck in Hueco Mundo— is Ichigo eating a Hollow right now???

The Shackling Prison was the place where the most heinous criminals resided, criminals kept for the rest of eternity to pay for their transgression against the Xianzhou Luofu. Made with floors that go as far above into the sky as well as far into the ground where the chilling cold permeates above all. A Borisin wolf war criminal and a Xianzhou native using the people for biological and alchemical experiments were few of the sinners held inside and yet, none were hated as much as the Vidyadhara that dabbled in immortality of The Abundance.
The Vidyadhara that damned their own fate for eternity.

Summary

So when his heat strikes after he runs out of suppressants, and it's a real one, his biology throws a wrench in his newfound plans of surviving until the others don't need him: His heat is lethal if he doesn't mate. He's convinced to get help, but that has some lasting side effects.
