Summary
After the deeply questionable experience of reincarnation, Yara came to one conclusion rather quickly. In the world of One Piece, a medical education was far more useful for forging licenses than for practicing legally.
At first, life as an underground doctor had seemed almost ideal. Quiet work. Good money. Minimal attention.
She treated pirates, smugglers, fugitives, and, on occasion, complete lunatics held together by stubbornness, poor impulse control, and what could generously be called luck. They paid well, rarely complained, and, most importantly, knew better than to ask unnecessary questions.
Yara extended them the same courtesy.
It was a simple arrangement: patch them up, take the money, stay out of their business, and keep as far away as possible from anything that smelled remotely like world-shaking conflicts or plot-induced insanity.
Honestly, it was a solid plan. Relatively safe, too.
At least until she discovered that world-famous maniacs apparently possessed an uncanny instinct for tracking down people with questionable reputations.
Seriously, fuck them all.

It should have been impossible unless Dragon was not only a shitty father but also had shitty tastes in women, but the body curled around him, rumbling in something that was half caught between a growl and a purr, felt too much like Luffy for Ace to be able to deny what has happening in front of his own eyes.

The dragon, because it was a dragon no matter how much Ace wanted to say it wasn't, rumbled back at him and yeah that was his little brother. His little brother who was a dragon.
Don't think of the implications Ace.
Who no one could try to kill now because everyone knew what it meant when someone had a dragon shift.
Anyone but Luffy probably.
"GARP!" echoed from somewhere and on this Ace at least could agree with that bastard fleet admiral. If Gramps knew about this he was going to punch him straight in the face.

Summary
The concept of luck is always a capricious notion, intrinsically engraved on the minds of those wronged by chance and mishappenings, doomed to follow a life where nothing is certain due to the whims of fate and the inevitability of fortune.
Thankfully, that was far from the case, for humans had agency over their actions, struggling against the binds existence threw at them and forcing meaning onto the most insignificant of things, while also disregarding the quagmire of powerlessness swallowing them all up. and that wasn't to say luck had no place in twisting events as it saw fit, in fact, humans were not born equal to the probabilities placed upon their path, not at all, people were born either graced with good fortune or cursed with atrocious misfortune.
Kaosu is a terminally ill kid and a user of the Raki Raki no Mi (Luck Fruit) who ends up crossing paths with another terminally ill kid, and chaos ensues.
Or: What if Law and Corazon weren't alone in their search for a cure? What if their first venture warranted not only a burning hospital but also a new companion in their journey?
