In one world, Katsuki had lost Izuku to Ochako.
In another, Izuku had lost his Kacchan during the Sludge Villain Incident and because of that, he became a villain.
After he was sent to another universe, Villain Izuku refused to go back. He didn't want to go back to a world without Kacchan. So what did he do? He killed himself.
What he didn't expect was to be sent to another universe which has Kacchan who was alive but brokenhearted.

Johnny whips around, eyes scrunched closed in frustration. “I appreciate it, but I’ve got it handled-“ he opens his eyes and immediately his tongue goes thick in his mouth, and he’s left staring dumbly at what he thinks is the most beautiful man he’s ever laid eyes on.
Said beautiful man smiles at him, and Johnny swears his heart skips. “Steve Harrington, and I’ve got a fair bit of babysitting experience if you need a hand.”
