Summary:

At approximately 02:13, a containment breach alert was triggered within D-Class Housing Sector █ following unauthorised movement between secured cells. D-████ was observed exiting Sector C via restricted corridors using a stolen Level 3 keycard.

Subject bypassed two (2) security checkpoints before being intercepted near Access Junction ██. Upon refusal to comply with verbal commands, D-████ was terminated on-site by security personnel at 02:19.

Following standard lockdown procedures, a sweep of D-Class Housing Sector █ was conducted. At 02:25, D-████’s assigned cell was opened. D-5136 was discovered inside.

 

Condition of D-5136:

D-5136 was located on the floor of the cell, unresponsive.

Observed condition included:

  • Significant cranial and facial trauma
  • Bruising around wrists and upper torso consistent with restraint
  • Partial state of undress
  • Presence of blood (confirmed to be from both D-5136 and D-████)
  • No keycard present within the cell

Medical personnel were dispatched immediately. Subject regained consciousness approximately ██ minutes later in the Site-91 Medical Wing.

 

Addendum A - Reconstruction of Events:

Based on security footage, recovered evidence, and post-incident interview with D-5136:

  • D-5136 exited her assigned cell at approximately 01:57 using a stolen Level 3 keycard
  • Subject proceeded directly to D-████’s cell
  • Entry was granted at 02:01
  • No further visual data is available due to D-████ destroying his cell’s camera earlier in the night
  • D-████ exited the cell alone at 02:12, exhibiting signs of distress and carrying the keycard used for entry

Addendum C - Corroboration:

Interviews conducted with multiple D-Class personnel confirm that D-████ had been interacting regularly with D-5136 over a period of approximately █ weeks prior to the incident. These interactions were described as unusually patient and non-aggressive compared to D-████’s typical behaviour.

Several subjects reported that D-5136 appeared hesitant initially but gradually became more receptive to D-████’s presence. No prior physical altercations between the two were reported.

 

Conclusion:

Incident Report 91-██-████-5136 demonstrates:

  • Exploitation of known behavioural patterns in D-5136
  • Security vulnerabilities related to unauthorised keycard acquisition
  • Increased risk associated with D-5136’s unsanctioned movement between cells

Recommendation:

  • Increased monitoring of D-5136’s movements and interactions
  • Restriction of cross-cell contact privileges
  • Review of all current guard personnel.

No disciplinary action is to be taken against D-5136.
